TERMS AND CONDITIONS FOR THE USE OF HR PORTAL
This HR Portal allows you, as the duly authorized representative of our Client,
to easily access your Company’s HMO membership information and enables you to view
relevant information of your enrolled employees and their dependents, such as their
names, gender, birthdate, membership status, civil status, as well as their utilization
records, including that of their dependents. As this platform requires the processing
of sensitive personal information, please take time to read the terms and conditions
in using this portal below.
PRIVACY NOTICE
DATA PRIVACY COMMITMENT. Cognizant of our obligations under the Data Privacy
Act of 2012, we commit ourselves to compliance with the principles of transparency,
legitimate purpose and proportionality. To this end, we always take commercially
reasonable steps to protect and maintain the confidential nature of your personal
information in accordance with its applicable privacy policies. Security measures
are also implemented to mitigate the risks of unauthorized disclosure.
PURPOSE. Please note, the processing of the information through this platform
shall be used soley for the purpose of providing you the HMO Services pursuant to
the Health Plan / Group Corporate agreement of your principal, our Client.
YOUR RESPONSIBILITY. Your access to this platform is based on your Principal’s
representation that you have the requisite authority. We shall not be responsible
for any misuse or unauthorized disclosure of any information available on this platform
arising from your sole negligence. Should you no longer have that authority to use
this platform, kindly inform us immediately.
WHAT INFORMATION YOU WILL HAVE ACCESS TO: As the duly authorized representative
of your Principal, you will have access to utilization report of all your Principal’s
enrolled employees covered by the Health Plan we provide. Said report contains the
amount of utilization, treatment and diagnosis. The grant of this access is based
on your Principal’s warranty that the consent of the data subjects allowing you,
on behalf of your Principal, to access said information, has been provided.
TO WHOM INFORMATION MAY BE DISCLOSED BY US. All information accessible through
this platform shall be treated as confidential. However, disclosure may be made
only to the following:
1. Data Subject. Upon request pursuant to their right to be informed.
2. Member’S Spouse/Partner/Immediate Family Member. Only when it is necessary to
protect your life as a patient.
3. The Principal Member. Whenever the member is a dependent-minor or a dependent
under guardianship, the principal shall be given access to your utilization record,
upon request. For all other cases, we will only disclose should you, if a dependent,
provide authorization allowing the principal member to have access to your records.
4. Government/the Courts/regulatory agencies. Only when required by law and or to
protect our legitimate interests.
ENFORCEMENT OF RIGHTS. If you have concerns regarding data privacy and the
enforcement the data subjects’ rights to be informed, to object, to access, to correct,

STORAGE. We store your information in our systems located in Makati City Philippines
for a period no longer than five (5) years. Storage may exceed beyond the period
only if allowed under the Data Privacy Act of 2012 or any applicable law.
